IMMUNIZATION INFORMATION

It is important to keep your children's immunizations updated. There are several new vaccines available. Check with your docotr regarding these. Immunizations are available through the following:

• Your doctor-Most PA based insurance companies will cover the cost of immunizations.
• Mercy Hospital Immunization Clinic-There is no charge, no appointment necessary. You will need to bring your child's shot record.
• PA Department of Health in Tunkhannock-Appointments are available on the 2nd Thursday of each month 9AM until 4PM. There is a $5 fee, payable by check or money order. You will also need to bring your child's shot record. Call for an appointment at (570) 836-2981.

MEDICATION POLICY

It is the policy of the Lackawnna Trail School District not to distribute medication of any kind without written permission of the physician or parent. This includes non-prescription medications, as well as prescribed medications. Students are not allowed to have medicine of any type in their possession. All medication must be given to the school nurse by the parent or guardian.

Please keep emergency contact information up to date. Current phone numbers are very important. Notify the school with any changes immediately. Update your emergency form as needed.
