As the school year comes to an end, it’s a great time to go over paperwork needed for the start of the next school year!!
If you are visiting your physicians over the summer, please reference the State Requirement schedule below to make sure you have the proper paperwork with you before going. The HS office also has paper copies available for pickup. All grade level paperwork will also be sent in summer mailings in August.
If your child has a specific illness that requires a care plan a paper copy will be sent in the 4th quarter report cards. Those in need of copies before then or a newly diagnosed individual can print forms from this page– to the right.
Grade K:
- Physical Exam upon entry & copy of up to date immunizations
- Body Mass Index (BMI), Height, Weight & Vision. -performed by School Nurses (unless otherwise requested by parent)
- Hearing Screening- performed by School Nurses (unless otherwise requested by parent)
- Dental- Private exam or School Dental exam in November**

- Physical- private exam or school physician in April – MUST HAVE PHYSICAL TO ATTEND 6TH GRADE FIELD TRIP
- MCV4, Tdap – usually given during 6th grade physical exam at private physician
** Dental and Scoliosis screenings may be performed by your private physician, if desired. Students who choose to have screenings done by their physician have until November 1, 2023 to return the completed forms to the nurse’s office. If the appropriate forms are not received by that date, the student will be screened by the school’s dentist and/or physician (unless the nurse is otherwise notified).
6th grade private physical exams are due by April 2024. Our school physician will visit at that time and screen any student who has not handed in a physical exam. If you do not want your child screened, please notify the school nurse or hand your exam in prior to April. Students who do not have physicals on file will not attend the school trip in May.
Must provide proof prior to day 1 of new school year.
Those Entering Kindergarten:
- 2nd MMR & Varicella, 4th DPT & Polio Immunizations
Those in 6th/ Entering 7th grade:
- Meningitis (MCV4)
- TDAP
